Please complete the form below and submit it to our registrar.. It is important to note that this information will be used to activate your Hockey Canada Registry and the address and contact information will be used to complete the tax credit receipt sent out by Otonabee Minor Hockey.

Prodcedure
Because of the manner in which the Hockey Canada Registry is set up, the first year of registration for a player in our system needs to be completed through our registrar. In subsequent years the player may access the HCR website and register directly.
